In order to help ensure that each active SIU member and pensioner receives a copy of the Seafarers LOG every month—as well as other important mail such as W-2 forms, pension and health insurance checks and bulletins or notices—a correct home address must be on file with the union.If you have moved recently or feel that you are not getting your union mail, please use the form linked on this page (PDF format) to update your home address.
Your home address is your permanent address, and this is where all official union documents will be mailed (unless otherwise specified).
If you are getting more than one copy of the LOG delivered to you, if you have changed your address, or if your name or address is misprinted or incomplete, please fill out the form and send it to:
